In-office Testing

Endocrinologists use specialized equipment such as our EAD imaging center to assist in the diagnosis, evaluation and prevention* of osteoporosis, the determination of body fat composition and the diagnosis thyroid disorders

The medical equipment at our on-site facility represents the most modern technology available.

Thyroid Ultrasound at the EAD Imaging Center

A thyroid ultrasound is a painless procedure that uses sound waves to generate images of your thyroid. Your doctor will recommend a thyroid ultrasound to examine the thyroid for abnormalities like cysts, nodules and tumors.

Bone Density Test at the EAD Imaging Center

The bone density test utilizes a form of imaging called Dual Energy X-ray Absorptiometry (DEXA) to measure bone strength (bone mineral density). The procedure is easy, painless, and quick, taking only about 10 minutes.

Moreover it has a high degree of safety because the exposure to radiation is much lower than that of a routine chest X-ray. DEXA technology is also used to measure body fat composition, providing information that is useful to patients who wish to improve their general fitness as well as certain serious medical conditions.

Office Policies and How-Tos

To Make An Appointment in our Dallas office, call 214-363-5535. When the automated phone system answers, press 2 to connect with the scheduler. To Make An Appointment at our Plano office, call 972-867-4658, then press 1 for scheduling.

We will call you with a courtesy reminder two days before your upcoming appointment.

Please be sure to have the following items with you when you arrive at our office:

  1. Insurance card(s)
  2. An up-to-date list of your prescriptions, over-the-counter medications and/or supplements, including dose and frequency
  3. Previous blood test results and any pertinent new information from other physicians that yet may not have been forwarded to our office
  4. Co-payment (due at the time of your visit)

Prescriptions

To obtain a refill for a regularly used medication, please request a prescription during your office visit. It may be necessary otherwise for you to come by our office to pick up your refill prescription. We cannot accept a fax or telephone prescription request from a pharmacy.

Cancellations

If you are unable to keep an appointment, please provide us with 48 hours notice prior to your scheduled visit. In the event of a sudden and urgent situation, please contact us as soon as possible. This will allow us to re-enter you into the schedule as well as to accommodate patients awaiting appointments or those who need to be seen due to illness or other special circumstances. There may be an administrative fee for missing an appointment without contacting our scheduler.

After-hours Assistance

In the event of serious illness or injury occurring outside of our regular office hours, call 9-1-1 or go to the hospital emergency room. For other medical concerns needing attention prior to the next business day, call the office and provide your name and phone number. You will be contacted by the physician on call.

Financial Policy

Endocrine Associates of Dallas (EAD) and Plano works with most major insurance plans. However, questions about coverage or benefits should be directed to your insurance company or checked in your schedule of benefits. Please be aware that any costs not covered by insurance are due at the time of your appointment. This includes co-payment and deductible. Your insurance plan may also require you to pay a portion (“co-insurance”) of various medical care fees. For information, check your schedule of benefits or contact your insurance provider. There is also a charge for clerical and administrative services which are not covered by insurance but may be requested by patients. Many require significant office staff time.

The fees range from $15 to $100 and include the following services:

  • Medical Records – copying for the patient (there is no fee for sending records directly to another physician) See the “Medical Records” section below.
  • Requests for “prior authorization” forms from an insurance company for coverage of medication or services (Patients can request such forms from the insurance company at no charge)
  • FMLA forms, disability forms, insurance appeals and letters to be used for legal purposes
  • Telephone consultations: frequent or lengthy calls
  • Growth Hormone start-up services: preparation of patient application and pre-certification.
  • There may be a service charge for missing an appointment without notifying our appointment scheduler. Please refer to the “Cancellations” section on this page.

Forms of payment accepted by our office are check, cash, insurance assignment and the following credit cards:

Medical Records

To obtain copies of your medical records, call the office to be connected with the Medical Records staff. EAD requires you to sign a Medical Records Release Form prior to forwarding your information. If you request copies of records to be sent directly to another physician, there is no charge. However, there is a nominal service fee required in advance for obtaining copies of medical records for any other purpose.

Due to the volume of requests for medical records we require 10 business days to complete these services. Therefore please notify us well in advance of the date that you will need to obtain or forward a copy of your records.

Diabetes Education Center

For over 30 years, the Diabetes Education Program at Endocrine Associates of Dallas and Plano has been nationally accredited by the American Diabetes Association (ADA). This accreditation is vital and ensures that our patients are given the most up to date information in the care of Diabetes.

Diabetes education, also known as diabetes self-management education (DSME) is a collaborative process through which people with or at risk for diabetes gain the knowledge and skills needed to modify their behavior and successfully self-manage their disease and its related conditions. Your visit in our Diabetes education center at Endocrine Associates of Dallas is a one-on-one educational experience.

We offer help and guidance in various areas of care, including:

  • Diabetes management
  • Cholesterol and triglyceride control
  • Weight management
  • Specialized diets for such conditions as:
    • Celiac disease
    • Kidney stones
    • Hypertension

  • Our office will need a completed “Patient Information and Consent” form for ALL new patients.
  • New adult patients will also need to complete the “Adult Health Questionnaire”.
  • If you are bringing a child for a first-time office visit, we will ask you to complete the “Pediatric Health Questionnaire” as well as the “Patient Information and Consent form”.

What to Expect During Your First Visit

Endocrinology involves evaluation of hormones. This testing requires blood. During your evaluation, obtaining a blood sample will be requested.

It is important that our patients be prepared for their visit. In addition to testing of blood and urine, it is important that our patients be prepared for a full examination.

  • Pediatric patients will have a complete physical examination to determine pubertal/developmental staging.
  • Men with concerns of testicular dysfunction often need genital and prostate exams.
  • Women with abnormal hair growth or breast secretions, may need evaluation of the breast and/or genitalia.

All forms are provided for new patients at our office. However, it does take a bit of time to complete them. For your convenience, you can download and print these documents from this website and complete them at home to bring to your first office visit. Due to legal requirements, please sign and date these forms in person during your office visit.
